If you are prone and at max or near max distance, no matter what shot you use, you can get off a number of shots (depending on your speed) before the creature automatically responds to you. The closer you are and the higher the posture, the shorter this time will be. Concealshot does not really change that, except that it prevents this auto response mechanism as long as you are hitting it with Concealshots. But you still pickup a creature TEF (temporary enemy flag) for that creature. So if it wanders too close to you, the chances are that it will notice you just because it became aware of you, and because you are an enemy (the TEF) it will attack, just like an aggressive creature would automatically attack you once you got too close to it and it became aware of you. Missing twice will allow the creature to become aware of you, and it will attack. Also, there is a chance, with any shot, that a small random bleed will occur (this likely has to do with the wound chance of the weapon) and when the first bleed damage ticks off (as with any bleed shot), the creature will automatically attack you.
But I look at these as mechanisms outside of Concealshot, so I would say, no, Concealshot will not fail in that sense, but it is not risk free. Get prone and get at your weapon's ideal range (preferrably 50m or more). Stack some Concealshots and then watch your target. If looks like it is going to get within 40m, stop shooting and setup again before resuming. If you see a bleed indicator on the target. Get ready to get up and use a different strategy because when the bleed ticks off, you will be aggroed. And as long as you are not detected, make sure you have enough Concealshots queued so as not to end up shooting the creature on autofire by accident.
And yes, it is a random pool attack.
If you miss a certain number of times or attack the target with a non-ConcealShot attack (this includes a bleed tick) you will be detected and attacked. You have to watch out as ConcealShot sometimes causes a one point bleed which will result in you being detected. The general numbers are 1 miss for standing, 2 misses for kneeling, and 3 misses for prone. You can reset this by using /Peace if you miss.
Also as has already been stated, if the MOB gets to close to you it will detect you and attack. So if possible set up about 55m or so. If it comes to close, use /Peace and move back and set up again.
For best results be prone and use the /Aim ability.
