Regarding note 17 (constantly respawning SMC Councilwoman) I could not determine which outpost he was talking about. It seems likely he was talking about the Trade Outpost. However, I could not locate what he calls "a set of monoliths (a la Stone Henge) that you will see on the overhead map" in my screenshot list.
